Direct Primary Care Membership Fees

Unity Health delivers primary care services based on membership to our practice. Enrolling in our practice requires a non-refundable, enrollment fee of $25 and a monthly membership plan that fits your situation. A monthly membership gives you 24/7 access to your provider via phone, text, email, video, etc., and unlimited office visits at no additional cost. Some Health Savings Accounts may allow the use of the funds to cover Direct Primary Care. We strongly suggest to check with your HSA administrator for specific details.

2 adult memberships + 2 child memberships FREE (12-17 years old), each additional child $5 + pay only 1

enrollment fee of $25

1 adult membership + $5 per child (12 – 17 years old) + pay only 1 enrollment fee of $25

Discounts (ALL PLANS)

Quarterly payment = 1 month free

Annual payment = 2 months free

Law Enforcement/Military/Veterans = 2 months free


Opioid Dependence Treatment

Non-refundable deposit to hold spot: $75 - this is applied to the initial visit

Initial visit: $375 minus the $75 deposit = $300 due at initial visit

Monthly visits: $250

Sick Visits: $30

Sports Physicals: $20
